Outpatient Rehabilitation for Stroke Survivors

Facilitates the independence of stroke survivors as they transition from the hospital and reintegrate back into their community. Provides outpatient rehabilitation services by an interdisciplinary team of allied health professionals.
Community Rehab Early Discharge Initiative (REDi) is delivered in outpatient clinics (attached to hospitals) in six Fraser Health communities. It is an outpatient service providing coordinated rehab to clients who have realistic, achievable functional goals, who no longer need to stay in the hospital and can be supported at home. It includes three components:
  • Central intake process that coordinates services and defines a clients’ unique care plan
  • Outpatient clinics delivering interdisciplinary rehabilitation Community reintegration programs such as TIME (Together in Movement and Exercise) classes and
  • Stroke Education classes

Referral Instructions: A referral is required in order to access the program. Referrals are accepted from physicians, allied health professionals, or home health. Fax referral form to Central Intake. (See form and fax number information below). Clients seen need to have a recent event e.g. surgery, neurological event, hospital admission or recent decline in function that puts them at risk for hospital (re)admission or further decline.
