Advance Care Planning is a planning process for adults at any age or stage. Healthy adults, those who have a new diagnosis, or chronic illness, as well as those who are facing serious or life limiting illness. Advance Care Planning means thinking about what matters most to you. It involves thinking about what a good day looks like, your values and wishes for present or future health care and then sharing your wishes or instructions with close family, friends and health care providers. It also means deciding who will speak for you if you can’t speak for yourself. It is a way to give those who matter to you the confidence to make decisions on your behalf when you are unable.
The Fraser Health Advance Care Planning Team is here to support you, and those that matter to you. They can answer questions about:
- documents/legalities
- how to start/continue advance care planning conversations
- what paperwork is needed
- how to decide and document about a substitute decision maker.
The team also provides public information sessions to assist people to engage in the Advance Care Planning process. If you are interested in attending or hosting an event, call or email.
