Day Program for Older Adults and Adults with Disabilities

Provided by Fraser Health

Offers supportive community group programs for older adults and adults with disabilities.
Offers personal assistance, supervision, and an organized program of therapeutic health, social, and recreation activities to seniors and adults with disabilities. Group activities vary by program, and may include therapeutic recreation, social activities, nursing assessments, and health monitoring. Also offers special lift-equipped bathing facilities for individuals unable to bathe at home independently. A mid-day meal is provided.

Participant eligibility to the program is determined through the local home health office. Some services are provided free of charge while others include a cost based on your after tax income.

Translation services can be made available if sufficient notice is given.

An assessment and referral from a Home Health professional are required to access this program. You can call to start this process, or any of the following can on your behalf: family member, hospital, family doctor, friend, neighbour, community group, or government agency.
- If you are NEW CLIENT contact the Home Health Service Line at 1-855-412-2121. This line is operated 7 days a week, 8:30am - 9:00pm, and 8:30am - 4:30pm on statutory holidays
- If you are a CURRENT CLIENT contact your local Home Health Office.
- Once you have contacted the Home Health Service Line, you will be assessed by a case manager. They will help you to develop a care plan that suits your needs. If home and community care services don't meet your needs, you can be directed to other community resources.

Virtual Day Program for Older Adults services are also available for those who cannot attend in-person programs. You can see, hear and talk to the Day Program for Older Adults team from the comfort of your own home.
