Community Pain Management Program - Fraser North South and East Pain Hubs
Pain Management
Provided by Fraser Health
- 2.5-hour session once a week for eight weeks
- Group education and self-management skill building
- Group physical rehabilitation activities
- Individual physiotherapy and occupational therapy care
- An extended pain team review, including a pain specialist and pharmacist for those who need it.
- Community-dependent access to Pharmacist, Social Worker, Pain Specialist and Addiction Medicine Specialist consults, as needed
- Assisted referral to more pain, community resources, mental health and substance use services, as needed.
- Fraser North (Tri-Cities) - Located in the Suter Brook Building in Port Moody, serving people living in the Burnaby, Tri-Cities, New Westminster, Pitt Meadows and Maple Ridge areas
- Fraser South (Surrey) - Serving people living in the Delta, Langley, Surrey, and White Rock areas
- Fraser East (Chilliwack) - Located at the Chilliwack Home Health Unit, serving people living in the Abbotsford, Mission, Chilliwack, Agassiz-Harrison, Hope and Fraser Canyon areas
To be eligible, patient must:
- Be 18 years or older
- Have chronic pain
- Be able to attend an 8-week program
- Be interested in self-management skill building
- Be open to group participation
Clients are referred to the program by substance use services or their primary care provider or the Fraser Health mental Health and Substance Use Services.
Telehealth Services:
- Initial consultation with a patient available via telephone. - Follow-up with a patient available via telephone.
- Booking & Contact Note: Clients will be booked directly by clerical staff for follow up appointments over the phone and/or in person.
- Advice to a health care provider available via telephone.
- Case conferencing with a health care provider available via telephone.
