Falls Prevention Mobile Clinic

1 on 1 fall assessments targeted to at-risk seniors living in the community.
Falls Prevention Clinics are multidisciplinary community clinics held virtually (via Zoom) or in-person throughout Fraser Health. Each week our team heads to a different community location (recreation centres, senior centres, libraries, etc.) to assess and provide recommendations to seniors at risk of falling. We also host presentations (in-person and virtual) for clients who are managing well but are interested in learning how to reduce their risk of falling.

In order to attend a clinic, clients MUST have an appointment. This appointment is a one-time session and takes approximately 2 hours to complete. Clients must be willing and able to participate in the process and consider making changes based on recommendations. An Action Plan or list of recommendations will be sent to the client and their primary care practitioner after the clinic.

Referral Instructions: Clients are able to self-refer to the clinic. However, health professionals should consider referring community-dwelling clients who are at risk of falling. Please see the referral form below with included instructions. Once completed, please fax the referral form (See fax number below).
